Reminds me of an incident in a Japanese processing plant for radioactive materials some years ago. I think some people were killed or got very sick at that one. that was probably the worst radioactive release incident since Chernobyl (orders of magnitude less severe of course).
This one at Knoxville certainly points out the importance of doing things right. Processing plant are getting to generate more incidents than power plant, I think people get more sloppy and take things for granted. Safety culture is of course a very strong thing in nuclear power plant operation, it is billed as the top priority and absolutely needs to remain so.
